If you’re new to functional medicine and have questions about how I would approach your child’s care, then the discovery call is the perfect place to start. The free discovery call is designed to learn about your child and the specific health and wellness challenge your child faces. It’s also a time for you to know more about me and my services. It’s important to determine upfront if we’re the right fit for one another and if my services align with your needs. Your call may last up to 20 minutes and allow you the time to ask questions regarding my services. The discovery call does not establish a patient-provider relationship. Therefore, no specific medical advice unique to your child’s situation will be given during this call. 

Functional Assessment

90 Minutes

If you’re familiar with functional medicine and ready to start, feel free to skip the discovery call and schedule your child’s functional assessment now.

We will explore your child’s symptoms and interconnectedness during the functional assessment. We will start working towards identifying the root cause. We will discuss your goals and expectations and how Life Care Health and Wellness Coaching can help you regain control of your child’s health. Each functional assessment includes the following:

• Extended appointment with the provider
  • Thorough functional health evaluation
  • Lab review through a functional medicine lens
  • An integrative, customized plan of care
• One health coaching session
• One appointment with a nutritionist

You will leave this one-time appointment with a clear action plan providing the clarity and confidence you need to help your child restore calmness, improve focus, and balance their moods so that they can reach their full potential and excel at home, in school, and in social settings.   

Membership

12 Months

Life Care Health and Wellness Memberships are for those who are ready to address their child’s health and wellness struggles through the functional medicine lens. Functional medicine is a specialty and does not take the place of your child’s pediatrician. Members must be under the care of a pediatrician, as Life Care Health and Wellness does not perform routine well-child exams or address acute illness. The following services are included throughout the twelve-month membership:

• Five additional provider appointments to use throughout the year
• Three additional health coaching appointments to use throughout the year
• Two additional nutritionist appointments to use throughout the year
• Instant messaging support via the client portal
• Access to advanced testing
• A 10% discount on supplements and other health and beauty products
• Unlimited access to any educational classes held throughout the year

At the end of the twelve months, members have the option to renew their membership.

Food Sensitivity Testing & Consultation (Children, Adolescents, and Adults)

KENTUCKY RESIDENTS ONLY

Have you ever wondered if the food you’re eating may be contributing to your health and wellness struggles? Even the healthiest of foods may be causing a problem if you are sensitive to them. Food sensitivities can be tricky because your body may react up to three days after eating the triggering food. It’s also possible you don’t react every single time. Food sensitivities often create low-grade chronic symptoms like headaches, GI symptoms, brain fog, skin rashes, acne, joint pain, fatigue, etc. Each consultation includes the following:

• Food Sensitivity Test (either at home or by a Vibrant America contracted lab)
• Interpretation by a licensed provider
• Extended provider appointment (45-minutes)
• Individualized plan
• Resources for additional follow up

Pantry Makeover

90 Minutes

Get ready to makeover your pantry! Practice skills like label reading and discover healthier alternatives to some of your family’s favorite foods. The Pantry Makeover is an excellent addition to any membership but can be purchased as a single item. Before a Pantry Makeover, attending the label reading workshop is highly recommended to maximize your learning experience.

Smart Shopping Trip

90 Minutes

Navigate the grocery aisles and read labels like a pro. Learn how to grocery shop to fit your family’s new dietary lifestyle all while staying within budget. The Smart Shopping Trip is an excellent addition to any membership, but you can purchase it as a single item.  Attending the label reading workshop is highly recommended before a Smart Shopping Trip to maximize your learning experience. You may buy the Smart Shopping Trip as a group (up to 5 people).
